Transscrotal Piercing: Meaning, Healing, Placement & Jewelry Types
Whenever you hear someone say a word like transscrotal, it is a pretty good bet that he/she is talking about the scrotum. The scrotum is, of course, the pride and joy of mankind and it largely gives our woodies the gumption to get hard and vertical and pump out crude at velocities most cannons would envy!
So, the topic today has to do with transscrotal piercings. There isn’t much information about it on the internet and we can’t fold our arms and let that stand! So, here’s a comprehensive overview of all things to do with transscrotal piercings.
When you are done reading this article, we can almost swear you could teach a class on the subject at Harvard or your local health center and that is as it should be! Here we go!
Transscrotal Piercings- A Definition
A transscrotal piercing is an extreme body modification that involves the scrotum being pierced from back to front, or from one side to the other. The testes are not affected by such piercings and the jewelry used doesn’t go through them. Some call transscrotal piercings a “scrunnel” –scrotal tunnel– and you will often find folks that insist on spelling it as transcrotal piercing.
Transscrotal piercings are a modern invention and not very common due to the risks involved. They are among the most striking of all extreme body modifications and some get such piercings and post videos of their experiences on XXX sites for wankers to gawk at and beat their meat to. And that’s okay because we all have different things that float our boats and so long as that’s legal then there’s no problem.
As for stimulation, well, that depends. Most of those that go for this kind of extreme body mod do it for purely aesthetic reasons. But there are a few who say their transscrotal piercings give them some joy of the pleasing and erotic kind.
Piercing Procedure
Transscrotal piercings are hella complicated and can take up to an hour. Needles are not normally used for such piercings, with scalpels and dermal punches being utilized instead. Needles CAN be used for transscrotal piercings, but they heal unusually slowly and the holes made cannot be stretched as wide as transscrotal piercings made with dermal punches and scalpels.
Anyway, the piercing procedure involves the aforesaid scalpel or dermal punch being used to create a side to side or front to back hole -fistula actually- in a tightly clamped scrotum. This hole is then sutured, front to back, and large or exceedingly large gauge jewelry is inserted.
Worth noting is that the scrotum is composed of 3 layers- the skin, muscles, and membrane. As a rule, only the skin is stitched, as suturing the other layers along with the skin will make healing exceptionally difficult. Also worth noting is that most complications during this kind of piercing develop due to improper stitching or the use of unsterilized equipment.
Possible Complications
A transscrotal piercing isn’t in any way like most other piercings out there. It is major, invasive, potentially traumatic, and more like surgery than a piercing. It is also dangerous and that is to be expected because this is the scrotum and its storehouse of sweet eggs that we are talking about here.
Due to the complexity of the procedure, infection is possible and liable to get serious very quickly. Also, if the hole created in the scrotum to hold the jewelry fails to do its job, then healing will be difficult and prolonged.
This might not be a complication, but transscrotal piercings are for life. While they might close up a little if there’s no jewelry in them, there will still be a visible hole down there.
Jewelry Used In Transscrotal Piercings
Once the ball bag is pierced, barbells and rings are usually inserted, with the gauge varying according to individual anatomy. But often, inert and flexible jewelry is used.
The fun usually starts when the piercing has healed. Once this is done, the hole can be readily and massively stretched, and then plugs and captive bead rings of almost any size can be inserted.
But since this kind of piercing is uncommon, people who get it often choose to assert their boldness and individuality by utilizing custom jewelry. This is usually flashy and made of precious metals like gold. So yeah, the fella sitting across you sure might have balls of gold and the only way you will ever know is if he agrees to a teabagging!
Transscrotal Piercing Healing And Aftercare
Healing takes from 4 to 8 months. It is recommended to abstain from sexual activity for at least a week and if possible till the wound has completely healed and to avoid all contact with the bodily fluids of others.
Wear loose clothing for as long as necessary and be careful of how you sit. And for Christ’s sake, don’t go horseback riding, biking, and the like for the first few months, and do not engage in strenuous activity that can jar or traumatize the area.
Keeping the piercing clean and dry is of the utmost importance and that means no poking around down there for curiosity’s sake, and no touching the area with dirty hands. Do not rotate the piercing, as doing so can introduce bacteria and other icky stuff into the healing wound. You can wash the area with mild antibacterial soap, but be careful not to overdo it.
You might need to wrap the piercing with a waterproof bandage when taking a shit, or your toilet water will be splattering your healing wound, with consequent infection an almost sure bet. Multiple daily cleaning with a saline solution will do wonders, no spraying of perfume or cosmetics in the area, and the application of lotion down there is forbidden.
There might be bleeding the first few days after a transscrotal piercing, but that is normal. But continuous bleeding that lasts more than a week could be a sign of a complication and you better ring up your piercer or the family doctor and tell him all about it. Do beware of bad smells, throbbing pain, and unusual tenderness, and act fast when these symptoms occur.
Also, unless a transscrotal piercing has completely healed, do not for any reason hop into a community pool, Jacuzzi, lake, or ocean. Considering the kind of filth and sewage in those bodies of water, you are better off just eating a shotgun! Avoiding chlorinated swimming pools is also recommended as chlorine can irritate wounds, while non-chlorinated bodies of water are usually too packed with bacteria, viruses, and even parasites for comfort.
Pros & Cons Of Transscrotal Piercings
Well, you gotta be a stone-cold OG to even think about and get such an invasive piercing. And that gets you some pretty big bragging rights.
Transscrotal piercings are also easily customizable and the peculiar anatomy of the scrotum means that stretching them is a breeze. They are an effective way to advertise your individuality and sexuality and you can get them for yourself, with no one but your piercer knowing you have an extreme body modification like that. They also do not affect sexual activity in any way, unlike piercings on the penis.
The cons include the fact that these kinds of piercings aren’t the best when it comes to giving sexual pleasure. The potential for complications is relatively high and this is not a piercing that can be performed in a rush or a few minutes.
Also, just as in most other kinds of genital piercings, getting a transscrotal piercing means that you are forever at increased risk of catching STDs and might have to make some lifestyle changes to accommodate the new you.
Conclusion
Transscrotal piercings are hardcore to an almost unfathomable extent and are assuredly not for everyone. They deliver in the aesthetics department and regardless of that still aren’t even close to being in the running for the most popular genital piercings out there.

Anyone want to help me get a Trans scrotal piercing
WOW! Always thought it might be really cool to have this type of piercing.
Between the healing time and finding a REALLY sterile piercer (Surgeon?)
makes me think twice about doing this.
I have had a trans scrotal piercing for years and years. Did it myself by suturing the front skin to the back then used a small, long barbell. The first couple tries resulting in an infection so had to start over.
I have stretched it considerably since then and it is now 1 1/4″. I use an acrylic tube cut to the appropriate length, held in place from sliding through by O rings. .
I find it extremely erotic when I put my penis through when jacking off; the up/down movement of the testicles then is great. The trans scrotal piercing used in this way acts like a cock ring at the base of the penis which further increased the pleasure.
The friends I have shown it to are awed.
I think they’re beyond awesome and extremely hot. In fact, I’ve been trying to find somewhere I can get one in my area of Southern California, but my googling for transscrotal piercing doesn’t usually generate the information I’m seeking, but I have encountered some appendage hardening pictures, If you have any tips on finding a piercer who does these I would welcome them.

the piercing easy and efficient.
He should be able to refer you to someone.
Thank you
Hi Doug: I just had my TS done by Allen Dabbs of Aura Piercing in Studio City. He’s very experienced, nice as hell and did an amazing job. I’m two weeks into mine and it’s healing incredibly well. Allen actually chose not to do sutures for me but does them often as well. Good luck if you haven’t already put the knife to your scrotum.
I have three of them. They were easy to get done, healed fast and look great, along with my 00G PA.
Hi Tom: I just got my TS two weeks ago and already love it even though it’s still healing. I didn’t understand how you have 3 TS and how that works/looks and am really curious to know/see what you have going on down there. Thank you.
I really have been thinking about getting one. My friend has one and his response to me was “Don’t”. His is now completely healed but the process to become healed was VERY rough. It got infected three times. However, he is now really very happy with it and loves it. Yeah… I am still thinking about it.
Thank you
HI there . Absolutely love the idea ……any one know any reliable surgeon who would do this in the UK ? ( I say surgeon rather than piercer ) .
John
Im also interested in a transcrotal piercing. I live on the east coast. Can someone recommend someone that could do it on this side?
Many, many years ago I did a TP for myself. I made a long oval two sided clamp to put the front and back together HOWEVER, I was too naive (dumb) to realize that suturing was mandatory! The pain and healing (not together) was extensive. Since then I’ve bought and used ball weights that feel good, and I’ve also self done and worn half inch by one incy ID circular barbell, half-a-drava behind my glans. It is not as convenient as a half inch SS bolt cut down to 3/4 inch, which I wear 24/7 and urinates better.
